Your First Visit

At the reception area you will meet our competent staff who will request that you complete paperwork (obligatory documents, policies, and medical information) and provide your insurance card prior to your consultation with the surgeon. It is best to arrive 10–15 minutes early to fill out these forms. Alternatively, you can download the below forms, complete them, and bring them with you to the office on the day of your visit.

Please understand that although you may have received a referral, we do need to obtain much of your medical information directly from you in order to accurately and completely evaluate your condition. If your situation is complex or has involved prior testing of any kind, please forward or bring that material to us so that your visit will be a productive one. If your x-ray studies were performed anywhere other than PRH, you may need to bring the actual films or images to your first visit with us.

After you meet with the surgeon and develop a plan of care, our staff will help to implement this plan by scheduling follow-up visits or procedures, obtaining pre-authorizations, and dealing with other insurance issues. Feel free to call us with any concerns, and we will do our best to help you.

Insurance

Atlantic Surgical Associates participates with most of the major insurance carriers. However, at this time we do not participate with any state Medicaid programs, UnitedHealthcare, Aetna, or MVP. Contact our office for updates on our insurance participation.

If your policy requires a referral, you must arrange for this through your Primary Care Provider. Without a required referral, your insurance company may refuse coverage, in which case you will be responsible for any charges at the time of this appointment.

Please be prepared to pay any co-payments, co-insurance, and/or deductibles at the time of service. Our office accepts Cash, Check, Mastercard, and Visa. Any unpaid balances after 60 to 90 days may be passed on to a collections agency at your expense.

Your insurance policy is a contract between you and/or your employer and your insurance company. Specific questions regarding coverage and benefits should be directed to your insurance company. Our staff works diligently with you and your insurance company to assure that they meet their obligations in covering payment for your medical and surgical care.
